If you browse through it, you will note that a long series of human oversight errors, some small, some glaring, contributed to the event, any one of which would have prevented it's occurrence. I don't believe humans can operate a system of such complexity reliably, at least until we all learn to drive our cars without hitting each other. It would make sense to have a remediation plan in place to address such situations in future. It can even be cost effective, especially when compared to the financial trauma BP and their suppliers have yet to experience fully.
